GROW Clinic & Referral Criteria The GROW Clinic manages failure to thrive using a multidisciplinary team based approach.  Under the direction of Hans Kersten, MD, the Medical Director, the team works with family members to address the multiple issues associated with failure-to-thrive.  Call 215-427-GROW (4769) with questions

GROW Clinic Referral Criteria

1. Height/ weight falls below 5% for age
2. Weight declines two major % over 6 months time
3. Weight < 3 % for age on two occasions
4. Weight < 80% of ideal weight for age

GROW Clinic Exclusion Criteria

1. Known metabolic syndrome (e.g. PKU) or organic disease (e.g. congenital heart defect).  These patients should be followed-up with their respective specialty clinic.

Need from referring physician before GROW Clinic Visit

1. Last 3 weight, height and/or head circumference measurements
2. Copy of the growth chart (front and back, please)
3. Complete physician referral form

GROW Clinic Discharge (Graduation) Criteria

1. Compliance of care with GROW Team Family Plan
2. Growth velocity improvement
