AMA Project for Integrating Racial and Ethnic Disparities Reduction into IHI’s Chronic Care Model

CHE has received support from the American Medical Association to develop a model for integrating racial and ethnic disparities reduction and language initiatives into the Chronic Care Model (CCM) developed by Dr. Ed Wagner and employed by the Institute for Healthcare Improvement (IHI). The final report will focus specifically on two chronic conditions, diabetes and cardiovascular disease, identifying specific language and cultural actions to reduce disparities in the context of the CCM, for application in physician practice settings. The Drexel-Health Partners of Philadelphia Seminar Series on Innovations to Reduce Disparities in Health Care

Sponsored by Health Partners of Philadelphia, the objective of this seminar series is to offer the opportunity for Drexel University faculty and students, and the broader professional public health and provider community in the city to hear from and engage national leaders on the dimensions of disparities and ways to improve quality health care for racially and ethnically diverse residents in the Philadelphia area. Social and Health Landscape of Urban and Suburban America

Sponsored by The Robert Wood Johnson Foundation, this project has produced a series of five reports using national sources of information – the US Census Bureau, the Centers for Disease Control and Prevention, the Federal Bureau of Investigation, and others – to document the social and health improvements and challenges occurring in the nation’s 100 largest cities and their suburbs. Supplemental tables and profiles of each city and suburb appear on the project’s website.


Establishing the Importance of Cultural Competence in Health Care Quality Improvement

Sponsored by the Agency for Healthcare Research and Quality (AHRQ), the primary objective of this project is to establish the validity of cultural competence as an essential component of health care quality by documenting current efforts and exploring the development of a common agenda for future work. The final report for this project should be available in October 2006.
